The short version

US 421 leaves Winston-Salem as a four-lane freeway and runs west past Lewisville and Yadkinville to Wilkesboro. From there it's a four-lane expressway that climbs the Blue Ridge to Deep Gap and Boone. [7]

As of October 2026, the two big work zones are at Yadkinville, where US 601 is getting roundabouts at the US 421 ramps, and in Wilkesboro, where NCDOT is turning signals on US 421 into U-turn intersections. [8][13]

Up top, the Blue Ridge Parkway bridge over US 421 at Deep Gap is under repair into spring 2027. Expect single-lane closures on US 421 under it. [14]

In Boone, NCDOT bars through trucks on US 421 between US 221/NC 105 and the NC 105 Bypass. If you're not delivering downtown, plan your route around it. [1]

No posted bridge sits on US 421 in NCDOT's Oct. 9, 2026 table, and there's no permanent weigh station on the route. The one state rest stop is the Wilkes County visitor center near Wilkesboro. [2][5][4]

Winston-Salem to Yadkinville: the freeway

Through downtown Winston-Salem, US 421 is Salem Parkway. It was rebuilt in concrete between 2018 and 2020, and the old I-40 Business name was dropped. Our Winston-Salem guide covers downtown, the Northern Beltway and the east side. [7]

US 421 splits from Salem Parkway at I-40 on the west side of town. Wikipedia notes there's no ramp from US 421 south to I-40 west, so check your route if you're coming from the west. [7]

Coming in on I-40 from the east? NCDOT bars trucks over 26,000 pounds GVWR with three or more axles from the left lane between US 158 (Stratford Road) and a mile west of I-74. [1]

Past I-40, the freeway runs northwest through the Hanes Mall area, Lewisville and Shallowford Road, then crosses the Yadkin River into Yadkin County. [7]

During the week of Oct. 26-30, 2026, DriveNC listed daytime shoulder closures on US 421 and its ramps from Jonestown Road to Shallowford Road, from 9 a.m. to 4 p.m. Travel lanes stayed open. [3]

Yadkinville work zones, as of October 2026

At Yadkinville, Exit 257 is US 601. NCDOT is building four roundabouts on US 601 (South State Street) from Sharon Drive to Lee Avenue, including at both US 421 ramps. Blythe Construction holds the $19.8 million contract. [8][9]

Traffic shifted into the roundabout pattern on the weekend of Aug. 28-29, 2026, and the old signals came out. NCDOT now expects to finish in fall 2027. [8]

When the ramps close at night, signed detours use Unifi Industrial Drive, Exit 256, and US 421. DriveNC listed the US 421 North off-ramp to US 601 closed from 9 p.m. Oct. 28 to 6 a.m. Oct. 29 for drainage work. [8][9][3]

DriveNC also shows US 601 cut to one lane each way in the work zone, and a lane shift at NC 67 with lanes as narrow as 9 feet. Our tip: if your stop is in Yadkinville, swing wide and take the roundabouts slowly. [3]

A separate job finished in late May 2026: two roundabouts on Speer Bridge Road, one at each US 421 ramp. Older GPS maps may not show them. [10]

Wilkesboro: new U-turn intersections

West of Yadkinville, US 421 crosses I-77 and passes the NC 268 exits on its way into Wilkes County. It stays a four-lane freeway to Wilkesboro, a stretch widened from 2000 to 2003. [7]

Inside Wilkesboro, US 421 becomes an expressway with signals. NCDOT is converting about 2 miles of it, from NC 16 to US 421 Business, into reduced conflict intersections. Branch Civil has the $81 million contract. [7][11][12]

At these intersections, side-street traffic can't cross US 421 or turn left onto it. You turn right, then use a signalized U-turn lane. Left turns off US 421 stay open in their own lanes. [12][13]

As of Oct. 11, 2026, the pattern is in place at Winkler Mill Road, US 421 Business and Addison Avenue, near the Lowes Foods center. NCDOT expects all work done in fall 2027. [12][13]

The ramp from US 421 Business north onto US 421 north is closed. The Curtis Bridge extension now links the two roads instead. [11]

Our tip: a U-turn in a 53-foot rig takes room. If a delivery puts you on the wrong side of US 421, check the U-turn spot on a map before you go. [11][12]

DriveNC also listed a rolling roadblock for utility work on US 421 near mile marker 282 in Wilkes County from midnight to 5 a.m. on Oct. 30, 2026. [3]

Up the mountain: Wilkesboro to Deep Gap

Leaving Wilkesboro, US 421 climbs gently at first, then steeply up the Blue Ridge escarpment to Deep Gap. We found no official grade figure, so go by the signs. [7]

The downhill run toward Wilkes County has a history. In 2019, a southbound lumber truck lost its brakes and hit the runaway sand pile just past the Watauga-Wilkes line, the Watauga Democrat reported. [16]

In 2018, another truck heading out of Boone lost its brakes near the county line and hit three vehicles in a work zone. It was going about 65 mph in a 30 mph zone, the paper reported, and the road was shut for more than five hours. [17]

Both times, traffic was turned back or detoured at Deep Gap onto US 221. Our tip: check your brakes at the top, pick a low gear before the descent, and don't ride the brakes. [16][17]

At Deep Gap, the Blue Ridge Parkway crosses over US 421. The Park Service began repairing that bridge on July 27, 2026, with single-lane closures on US 421 below it until the work ends, expected in late spring 2027. [14]

Just past Deep Gap, US 221 splits off north toward West Jefferson. For Parkway rules and fall crowds, see our leaf season guide. [7][14]

Boone and west toward Tennessee

Near Boone the expressway ends and US 421 becomes a four-lane boulevard. It then runs with US 321 and NC 194 through downtown on King Street. [7]

NCDOT's table bars through trucks on US 421 from US 221/NC 105 to US 321 (Hardin Street), and from Hardin Street to the NC 105 Bypass. The ban is on through trips. Our tip: for a downtown delivery, ask the receiver which approach they use. [1]

Watch the side roads too. NC 194 is closed to tractor-trailers from SR 1117 to US 321/US 421 west of Boone, and from US 221/421 to the Ashe County line. [1]

US 221 south from Blowing Rock is also off limits to tractor-trailers between US 321 and the Caldwell County line. Our winter guide covers the Blowing Rock routing. [1]

West of Boone, US 421 and US 321 share a two-lane road near Vilas. NCDOT's project R-2615 would improve about 3.5 miles of it, from Vilas to the NC 105 Bypass, but construction isn't planned until 2040 or later. [15]

Past Vilas, US 421 is two lanes to the Tennessee line near Zionville and on to Mountain City. Our tip: it's a slow mountain road, so check DriveNC for closures before you commit to it. [7][3]

Truck rules, posted bridges and scales

NCDOT's truck restriction table, dated March 5, 2024, has no US 421 rows in Forsyth, Yadkin or Wilkes counties. The only ones on this route are the two Boone through-truck bans. [1]

Nearby, Caldwell County bars tractor-trailers on NC 268 from US 321 to the Wilkes County line. Our tip: don't use NC 268 off US 421 as a shortcut to Lenoir. [1]

NCDOT's posted bridges table for primary routes, dated Oct. 9, 2026, lists no bridge on US 421. The only Wilkes entry is on Stone Mountain Road, well north of the route. Secondary roads are mostly not in that table, so watch for signs. [2]

NCSHP lists eight permanent weigh stations, and none is on US 421. The closest are on I-77 near Mount Airy and on I-40 west of Statesville. [5]

That doesn't rule out checks. State law requires trucks with a GVWR of 10,001 pounds or more to stop at temporary weigh sites when signs say so, and an officer can send you to a scale within five miles. [6]

Rest stops and freight

DriveNC lists one state stop on this run: the Wilkes County visitor center beside the northbound lanes of US 421, about 3 miles east of Wilkesboro. It has 17 truck spaces and showed as open on Oct. 28, 2026. [4]

Opened in 2009, it was billed as North Carolina's first environmentally friendly rest area. DriveNC lists no state rest stop on US 421 in Forsyth, Yadkin or Watauga counties. [4]

Our tip: take your break at Wilkesboro before the climb, since DriveNC shows no other state stop on the way up to Boone. [4][1]

One big shipper is Tyson. Its Wilkesboro plant had about 2,500 workers in 2024, when it cut jobs while shifting toward fully cooked products, Manufacturing Dive reported. Confirm dock hours before you deadhead in. [18]
